Once you've completed the order form, you have two options:

(A) Print it out and mail it to the address below along with your photographs and check
or
(B) Attach your images and press submit to send your order via email. Send check by mail or use PayPal.

Please allow at least 6 weeks for completion of your portrait. You will be notified when the portrait is finished. A DEPOSIT OF 50% of the order is required before the portrait can be started. The balance is due upon receipt of the finished work. Deposits can be made by personal check, or by PayPal.

1. Debbie can use her painting of my dog to create items to sell to everyone.
2. Debbie can use her painting of my dog, but only in a private store for me.
3. I don't want my dog's image on gifts and clothing.
